This workbook was created to help people with disabilities talk openly and clearly about end-of-life planning when facing a serious illness or life changing event. It is designed to help friends, family members, service providers and health care professionals understand the values and wishes of the individual. It guides people on how to communicate difficult emotions and thoughts. Emphasis is on a person’s quality of life and supporting them through their end-of-life journey.
