This workbook shares personal stories of adults with disabilities to show the possibilities of community living. The chapters focus on a person-centered and team approach to planning for future housing transitions. There are checklists to help identify a person’s wishes, build social connections, and help individuals ask the right questions when finding resources. This project was funded by the Arizona Developmental Disabilities Planning Council. The workbook is available in English and Spanish.
