This workbook was an outcome of the Aging and Transitions Project, funded by the Administration for Community Living. The workbook serves as a guide for caregivers as they explore future planning with family members with a developmental disability. The chapters are designed to facilitate communication about desires, goals, and plans. It focuses on the planning process, health communication, living options, community living, financial planning, and legal considerations. It is available in English.
